Job summary

Compassus is seeking a Hospice Registered Nurse / RN Case Manager to support patients at home and in the community. You will coordinate holistic care, assess needs, and guide families through the treatment journey with compassion and expertise.

You will build trusting relationships, educate caregivers, and collaborate with the care team to ensure comfort, dignity, and quality time with loved ones in a supportive, inclusive environment.

Qualifications

• RN license in the state you work
• Two years of RN experience; hospice experience preferred
• Current driver’s license
• Current CPR and BCLS certification

Responsibilities

• Enable patients to spend quality time with loved ones and activities they enjoy.
• Build trusting, compassionate relationships with patients and families.
• Educate caregivers on care for loved one at home.
• Collaborate with the care team to provide comfort, peace, and dignity.
