Job summary

Suncrest Hospice is seeking a registered nurse to provide high-quality hospice care to patients in their homes or facilities within the Sacramento area. The role includes admissions, patient visits, and after-hours on-call responsibilities on a 7-on/7-off schedule.

Applicants must hold a current RN license in good standing, have admissions and on-call experience preferred, and be able to work closely with a multidisciplinary team of nurses, social workers, chaplains, and other staff to deliver

Qualifications

• Current Registered Nurse license in good standing.
• Admissions and on-call experience preferred.
• Works well within a team with nurses, social workers, and chaplains.
• Ability to travel across the service area with reliable transportation.

Responsibilities

• Provide care and services for hospice patients at home or facilities.
• Perform hospice admissions and patient visits as needed.
• Conduct nurse visits for symptom management, end-of-life care, and admissions.
• Answer calls from patients and families; communicate with care team.
