A Day in the Life of a Hospice Registered Nurse (RN)

As an RN, you play a key role in assessing the needs of our patients to create and implement their plan of care. This position is responsible for serving as case managers and providing individualized goal‑directed nursing care through the use of the nursing process in accordance with physician's orders. Responsibilities include but are not limited to the following:

  • Utilizing the nursing process, assess patient/caregiver needs in order to provide quality care to assigned patients.
  • Develop and communicate a realistic plan of care for each patient.
  • Serve as Case Manager for assigned patients and actively manage case loads and visit times in accordance with agency productivity standards.
  • Implement medical plan of care, nursing interventions, and procedures as necessary to ensure safe, effective, quality of care for patients.
  • Stay up‑to‑date on a variety of High‑Tech skills; IV use and maintenance, wound VAC, complex wound care, etc.
  • In accordance with LPN scope of practice, work collaboratively with LPNs to ensure that the plan of care is followed, delegate tasks and ensure patient needs are met.
  • Consistently review documentation, annotate visit notes and act on recommendations in accordance with department policies and procedures.
  • Evaluate a variety of aspects of implemented plan of care including, but not limited to identified problems, care provided and patient's responses, to meet patient/caregiver needs and ensure overall quality of care delivered.
  • Maintain up‑to‑date and accurate documentation of nursing care provided to ensure effective integration of information for use by the health care team to ensure ongoing and continued quality of care.
  • Perform patient and caregiver education to ensure informed patient decisions can be made, including, but not limited to, medication side effects, treatment options, discharge instructions, reportable conditions, ranges of services, etc.
Qualifications for a Registered Nurse (RN)
  • Registered Nurse (RN) license
  • 2+ years of nursing experience is preferred
  • Have self‑direction and initiative, and be able to work collaboratively with colleagues within the agency.
  • Be experienced with electronic medical records.
  • Willingness to work a flexible schedule, including evenings, weekends, and on call for both hospice and home care patients.
  • Exceptional bedside manner and communication skills.
  • Compassionate and friendly demeanor.
  • Valid Vermont Driver's License and car insurance.
