Zusammenfassung

Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate all aspects of in-home patient care and liaise with other medical disciplines. You will perform admissions, assess eligibility for home care services, and create comprehensive care plans tailored to each patient.

The role requires a nursing degree (Diploma/ASN/BSN), an active RN license, CPR, and reliable transportation for home visits. Nebraska-based, full-time with benefits and opportunities for professional growth.

Qualifikationen

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One year of RN experience; two years preferred.
• CPR certification and hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient's home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient's eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.

• One-year's experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
