Job summary

Proforma Health Inc. delivers compassionate end-of-life care to patients in their homes, providing skilled nursing while supporting dignity and comfort.

Perform patient assessments, develop individualized care plans, and manage pain and symptoms using evidence-based hospice practices. You will coordinate with physicians, social workers, and spiritual care providers to educate patients and families and document care in EMRs with strict regulatory compliance.

Qualifications

• Current RN licensure and clinical knowledge of hospice and end-of-life care.
• Strong assessment, care planning, and symptom management skills.
• Excellent communication and compassionate, patient-centered approach.
• Ability to work effectively in an interdisciplinary team.
• Proficient EMR documentation and commitment to compliance.
