Job summary

Optum's Heart n Home Hospice in Bend, Oregon seeks a compassionate Registered Nurse Case Manager to coordinate Plan of Care for hospice patients. You will evaluate care needs, document progress, and collaborate with the IDG to deliver high-quality hospice services.

The role requires an unrestricted RN license in Oregon, valid driver's license or reliable transportation, and strong clinical skills. A $5000 sign-on bonus is available for external candidates; CPR certification is preferred.

Qualifications

  • Must hold an unrestricted RN license in the state of practice.
  • Must have a valid driver’s license or access to reliable transportation.
  • CPR certification preferred; 1+ years of clinical experience beneficial.

Responsibilities

  • Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
  • Identifies patient/family needs and reassesses every 15 days.
  • Initiates and coordinates the plan of care with accurate clinical notes.
  • Documents problems, goals, interventions, and responses to care.
  • Collaborates with patient/family, attending physician, and IDG to provide daily care.
  • Capable of working in diverse home environments.

As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.Primary Responsibilities

  • Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
  • Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days
  • Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
  • Documents problems, appropriate goals, interventions, and patient/family response to hospice care
  • Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

  • Current unrestricted RN licensure in the state of practice
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
Preferred Qualifications
  • Current CPR certification or ability to complete within 90 days of hire
  • 1+ years of clinical experience
