Job summary

Summa Home Health and Hospice LLC is seeking a Registered Nurse (RN) to provide compassionate end-of-life care in patients' homes within the Akron/Canton area. You will work autonomously as part of an interdisciplinary team, delivering one-on-one patient care at the top of your license.

The role emphasizes plans of care, family education, and coordination with physicians, with PRN shifts across days, evenings, and weekends.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting is preferred.
• Hospice experience is desired.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care.
• Individualize the plan of care and make adjustments as needs change.
• Minimize Patient Risk: Update all doctor orders in patient chart and administer medication as prescribed by the physician.
• Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
• Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings.

HOW YOU'LL WORK:

You’ll oversee the overall care of the patient from beginning to end of life. You will observe and monitor the patient to determine if the level of care being provided meets the patients’ needs.
