Job summary

The role reports to the Medical Director and focuses on maintaining high clinical standards in hospice services. Requires California medical licensure, at least two years of supervisory experience in hospice or palliative care, and no recent disciplinary actions.

Qualifications

  • Doctorate degree in allopathic or osteopathic medicine.
  • Board certification in Hospice/Palliative or related field preferred.
  • Current unrestricted medical license in California.
  • NPI and DEA registrations required.
  • Willingness to travel 15–25% as program needs dictate.

Responsibilities

  • Implement and oversee patient Plans of Care to meet medical needs.
  • Assess and reevaluate patient medical needs regularly.
  • Supervise and communicate with all hospice providers.
  • Assist in establishing and reviewing patient Plans of Care.
  • Serve on the IDG and contribute to patient-focused care planning.
  • Attend and conduct clinical training sessions.
  • Assess hospice eligibility and complete certifications as needed.
  • Act as a medical resource and liaison for IDG, attending physicians, and community providers.

Overview

Expand Access. Build Partnerships. Transform Care as a Hospice Physician Designee.

We’re looking for a Hospice Physician Designee to join our compassionate care team. Reporting to the Medical Director, you will provide direction and oversight for patient care, ensuring that all plans of care are implemented and medically appropriate. This role is critical in guiding hospice care teams across multiple locations and maintaining the highest standards of clinical excellence.

This is a job notification only. The Hospice Physician Designee will not be employed by Gentiva. The Hospice Physician will invoice Gentiva for services rendered

MUST have per state of California Regulations:

Current valid license as a physician and surgeon issued by the Medical Board of California or by the Osteopathic Medical Board of California and

A minimum of two years of full-time supervisory or managerial experience in a hospice, home health agency, or providing palliative care to patients within the last five years and

No disciplinary action taken against them within the last seven years and

must not have concurrent employment with other hospices

Essential Functions
  • Implement and oversee patient Plans of Care to meet medical needs
  • Assess and reevaluate patient medical needs regularly
  • Supervise and communicate with all hospice providers, including contracted and employed physicians
  • Assist in establishing and reviewing patient Plans of Care
  • Serve on the Interdisciplinary Group (IDG) and contribute to patient-focused care planning
  • Attend and conduct clinical training sessions
  • Assess patient hospice eligibility and complete terminal illness certifications/recertifications when needed
  • Act as a medical resource and liaison for the IDG, attending physicians, and community medical providers
  • Facilitate meetings with patients’ family members and provide guidance for quality care
California Regulations

Current valid license as a physician and surgeon issued by the Medical Board of California or by the Osteopathic Medical Board of California and

About You
Qualifications
  • Doctorate degree in allopathic or osteopathic medicine from an accredited medical school.
  • Board certification in a specialty recognized by ABMS or AOA-BOS, preferably primary care.
  • Current and unrestricted medical license in the practicing state.
  • Valid National Provider Identifier (NPI) and DEA registration for controlled substances.
  • Hospice and Palliative Medicine board certification and/or Hospice Medical Director Certification Board certification preferred.
  • Current Medicare Provider Enrollment (PECOS) and Medicaid certification if required.
  • Ability to travel 15-25% based on program needs.
  • CPR and TB clearance required.
