Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy in a home setting serving Lakeland, FL. Help older adults maintain independence and quality of life through compassionate, evidence-based care.

Qualifications

• Must hold a Florida PT license.
• Degree from an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate).

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
