Job summary
BAYADA Hospice is seeking an experienced Per Diem Physical Therapist (PT) for hospice visits in Pennsauken, NJ. The role serves adult and geriatric clients across Camden and Burlington counties on a per-visit basis.

Qualifications
- One year of clinical PT experience is required.
- Licensed Physical Therapist in NJ.
- Graduation from an APTA-approved program or equivalent.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational and problem-solving skills.
- Solid computer skills; EMR experience preferred.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements for mobility and safety.
- Develop and implement individualized care plans with therapeutic exercises.
- Continuously assess and revise the PT care plan with the multidisciplinary team.
- Educate clients and families on rehabilitative care to promote independence.
- Document observations and interventions using a touch pad tablet.
