Job summary

Shalom Hospice is seeking a Per Diem Nurse Practitioner to complete Medicare-required Face-to-Face visits for hospice patients in their homes across the Nashville area. The NP will perform timely F2F recertification visits, document findings, and coordinate with the IDG to support eligibility and care planning.

The ideal candidate holds TN NP license with prescriptive authority, board certification in AGNP or family practice, at least 1 year hospice experience, strong documentation and time

Qualifications

• Active Nurse Practitioner license in Tennessee with prescriptive authority.
• Board Certification in Adult-Gerontology, Family Practice, or related specialty.
• At least 1 year of hospice or palliative care experience.
• Strong documentation and time management skills.
• Reliable transportation for community-based travel across the Nashville area.

Responsibilities

Job Summary:

We are seeking a detail-oriented, responsive, and empathetic Per Diem Nurse Practitioner (NP) to complete Medicare-required Face-to-Face (F2F) visits for patients on service or entering their third or later benefit period. As a key member of our interdisciplinary team, you will conduct timely and compliant F2F hospice recertification visits, document clinical findings, collaborate with the IDG, and maintain schedule flexibility.
