Job summary

PruittHealth Hospice is seeking a Hospice Palliative Care Bridge Nurse (LPN/RN) to support patient care in home or care facility settings. You will coordinate referrals, support the Nurse Practitioner and Medical Director, and schedule visits to advance goals of care.

This role requires an active RN or LPN license in the state of practice and at least three years of nursing experience; hospice or home health experience is preferred. CHPN is a plus.

Qualifications

• Active RN or LPN licensure in the state of practice.
• Minimum of three (3) years of nursing experience.
• Palliative care, hospice, or home health care experience preferred.
• CHPN (Certified Hospice and Palliative Nurse) preferred.

Responsibilities

• Take and process palliative care referrals.
• Provide support to the Palliative Care Nurse Practitioner and Medical Director.
• Schedule visits to support goals of care as ordered by the MD.
• Coordinate and communicate patient care goals between providers.
• Make calls to present the palliative program and obtain consents.
• Inform attending physician of assessment and reassessment findings.
• Complete documentation accurately and timely per policy.
• Inform palliative team of new admissions and discharges.
• Coordinate referrals using the referral process.
• Serve as liaison for medical directors and nurse practitioners to enhance effectiveness.
• Run billing report and address validation holds/billing errors.

KEY RESPONSIBILITIES:

Qualifications

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS:

• Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN)licensure in the state of practice
• A minimum of three (3) years of nursing experience
• Palliative care, hospice, or home health care experience is preferred
• CHPN (Certified Hospice and Palliative Nurse) is preferred
