Job summary

ATEX Healthcare Home Health and Hospice is seeking an experienced Registered Nurse to provide patient‑centered home health services in the Corpus Christi area. You will perform comprehensive assessments, develop and manage care plans, and coordinate with physicians and staff to ensure high‑quality, safe patient care.

The role requires a current RN license, CPR certification, and at least two years of home health or public health experience.

Qualifications

• Active RN license in the state
• Graduate of an accredited school of nursing

• Ability to work with clinicians, develop home plan of care, understand third‑party reimbursement, negotiate payment rates
• Self-directed with minimal supervision
• Excellent observation, verbal and written communication
• Must be a licensed driver with insured vehicle

Responsibilities

• Assumes case management for individual patients/families with comprehensive physical assessment
• Pain assessment and management and symptom control for terminal illness
• Weekend on‑call visiting staff nurse
• Complete documentation timeliness (assessments, care plans, notes)
• Provide weekend coverage; assist during nursing absenteeism
• Report to Patient Care Coordinator and work under attending physician
• Monitor medications for interactions and adverse effects
• Conduct full medical history, family support, home enviro and safety checks
• Coordinate and manage nursing services: records, supplies, infection control, care plans, scheduling, training
• Communicate with BM and staff to review issues and share feedback
• Maintain regulatory compliance and participate in staff meetings
• Adhere to company policies and perform other duties

GENERAL PURPOSE:Ensure staff and other support resources are appropriately utilized and a high level of interdisciplinary teamwork is maintained through delegation of duties.QUALIFICATIONS:Registered nurse with current licensure to practice professional nursing in the state.Graduate of an accredited school of nursing.Maintains a current CPR certification.Minimum of two (2) years' experience, at least one (1) of which is in the area of public health or home care nursing.Demonstrates an ability to work with other health care clinicians, development of home plan of care, knowledge of third-party reimbursement, and ability to negotiate payment rates.Self-directed and able to work with minimal supervision.Demonstrate excellent observation verbal and written communication skills.Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.Must be capable of performing the job functions of this position with or without accommodations.ESSENTIAL JOB FUNCTIONS:Ensures quality and safe delivery of services, within the guidelines of accepted nursing practice and state and local law.Assumes Case Management responsibilities for individual patients/family including:Comprehensive Physical AssessmentPain Assessment and management.Symptom control management as it relates to terminal illness.Weekend On call visiting Staff Nurse.Completes required documentation (i.e. assessments, care plans, clinical notes, agency tracking forms, etc.) in a timely manner in accordance with agency policies.Provides weekend coverage and may assist during times of excessive nursing absenteeism.Is directly responsible to the Patient Care Coordinator and works under the direction of the patient’s attending physician.Informs the BM of unusual or potentially problematic patient/family issues.Checks any medications the patient may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies and contraindicated medication.Conducts a complete medical history, family support systems, home environment and safety issues.Coordinate, implement and manage administrative functions and areas of programs related to nursing services which may include: documentation or medical records, nursing supplies, ancillary supplies or services, agency Pl; Care Plans; Infection Control; Skin Care Program; Special Programs; Staff Scheduling 'Selection, Training, Orientation or related functions.Communicates regularly with the BM and other staff members to review problems or unique issues from weekend visits, share professional support, and exchange feedback aimed toward enhancing professional growth.Maintain a working knowledge and ensure compliance with all governmental regulations.Participate in Nursing Staff Meetings as requested.Adhere to all of the Company's policies and procedures.Perform other duties as assigned.The above statements are only meant to be a representative summary of the major duties and responsibilities performed by incumbents of this job.
