Job summary

Dignity Home Health and Hospice is seeking a compassionate Registered Nurse (RN) to join our South Jordan-based team as a Home Health/Hospice RN Case Manager. You will coordinate patient care, collaborate with families and the interdisciplinary team, and ensure the individualized plan of care is effectively implemented across all visits.

Ideal candidates have strong clinical skills, excellent communication, and a commitment to ethical, patient-centered care.

Qualifications

  • RN license required or equivalent nursing licensure.
  • CPR certification required or preferred.
  • Experience in home health or hospice is beneficial, willingness to train suitable candidates.

Responsibilities

  • Plan, coordinate, and provide compassionate home health/hospice care.

Position Description

The RN Case Manager is an essential member of our interdisciplinary team, working collaboratively to address the physical, medical, emotional, psychosocial, and spiritual needs of patients and their families.

In this role, the RN coordinates all aspects of patient care, including ongoing assessments of the patient, family, and caregivers, while ensuring the individualized interdisciplinary plan of care is effectively implemented and updated as needs change.

Key Responsibilities
Patient Care & Nursing
  • Plan, coordinate, and provide compassionate hospice care in accordance with each patient’s individualized plan of care.
  • Collaborate closely with patients, families, caregivers, physicians, and interdisciplinary team members to ensure comprehensive needs are addressed.
  • Monitor changes in patient condition and communicate with the attending physician and Hospice Medical Director when adjustments to orders or the plan of care are needed.
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care.
  • Develop, coordinate, and update individualized care plans based on the patient’s changing needs.
Documentation & Care Coordination
  • Maintain complete, accurate, and timely clinical documentation.
  • Keep patient care plans current and reflective of the patient’s condition and needs.
  • Participate in interdisciplinary group (IDG) meetings and case conferences.
  • Attend bi-weekly interdisciplinary team meetings to promote continuity and coordination of care.
Patient & Family Education
  • Educate patients and families about the hospice philosophy, plan of care, disease progression, and the dying process.
  • Offer compassionate guidance and support as patients and families navigate healthcare decisions.
  • Obtain and document relevant medical histories from patients and/or family members, particularly information related to the patient’s current condition.
  • Provide education and support to patients, families, caregivers, and referral partners.
Resources & Communication
  • Identify and coordinate appropriate resources and referrals, including occupational therapy, physical therapy, pharmacy services, medical supplies, durable medical equipment, and other specialized services.
  • Maintain clear and effective communication with patients, families, physicians, and members of the care team.
  • Provide thorough written and verbal reports to support continuity of care.
Technology
  • Utilize electronic health records (EHR), email, computers, and mobile/cell phone technology as required to document, communicate, and coordinate patient care.
Schedule

On-call not required, unless desired (we have a separate on-call team)
