Job summary

Three Oaks Hospice is seeking a Nurse Practitioner to collaborate with physicians and the palliative care team to deliver compassionate care to chronic and seriously ill patients and families.

The role focuses on evaluating, planning, and educating patients and families about disease processes, end‑of‑life care, and treatment options, with responsibilities across care coordination and care delivery. Strong communication and clinical skills are essential.

Qualifications

  • Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
  • Current state RN and APN license and CPR certification.
  • Minimum 2 years’ experience as an APN in clinical care setting preferred; hospice and home health preferred.
  • Certification in a specialist area preferred; (e.g., Hospice and Palliative Nursing (CHPN) or Geriatrics)
  • Excellent observation, verbal and written communication skill, problem solving skills, nursing skills per competency checklist.
  • Reliable transportation, valid driver’s license, and current auto liability insurance coverage.

Responsibilities

  • Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
  • Consults with and assists in the IDG’s development of patient/family plan of care to assure quality care is provided.
  • Consults with and educates the patient/family regarding disease process, self‑care techniques, end‑of‑life care. Provides training to other staff as needed.
  • Performs and documents visits to patient/family based on plan of care and patient/family need.
  • Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
  • Consults with and provides education to other IDG members on a prn basis for clinical issues relating to palliation.
  • Actively participates in the IDG. Communicates effectively, timely, and appropriately at all times.
  • Informs physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.

POSITION SUMMARY:
