Job summary

A Path of Care Hospice of Lawton, OK is looking to hire a PRN Nurse Practitioner. This role offers flexible hours and meaningful work within a home health care setting, with an emphasis on compassionate patient care and collaboration with physicians and the care team.

As part of the team you will perform visits, examinations, Face to Face encounters, and document care plans. The position supports professional growth in a supportive environment, with opportunities to build a long-lasting career

Qualifications

• Must be a Nurse Practitioner legally authorized to practice in the State of Oklahoma and have a license free of sanctions.
• Work experience in geriatrics, palliative medicine or other relevant field of medicine.
• Work experience in a certified hospice program experience preferred.
• The Nurse Practitioner works in the field 95% of the time with frequent bending, stooping, sitting, standing and walking.

Responsibilities

• As a Nurse Practitioner, you perform visits, physical examinations, Face to Face visits and documentation.
• Collaborate with the Administrator/Director and Medical Director and other health professionals in developing formal patient care policies.
• Help ensure care is delivered in accordance with acceptable standards of practice and meet patient/family needs.
