Job summary

Ohio Living is seeking a Nurse Practitioner to provide preventive and palliative care, plus education, to patients in home health and hospice settings. The role uses standard software and our clinical-financial system, ensuring compliance with state, federal, and agency guidelines.

You will take histories and perform exams, manage common illnesses and chronic conditions, order tests, counsel patients, refer to specialists, and maintain accurate documentation while delivering excellent customer

Qualifications

• Current unencumbered Ohio NP license.
• Prescriptive Authority or Externship to Prescribe.
• Two years of home health or hospice experience.
• Proficiency with Windows and MS Office; EMR experience preferred.

Responsibilities

• Care Management - 100%: Conducts medical and social histories and exams.
• Treats acute illnesses and chronic conditions, prescribing medications.
• Orders and interprets labs and imaging.
• Provides information and counseling on health maintenance and palliative care.
• Refers patients to other health professionals as needed.
• Communicates with staff, patients and families to optimize outcomes.
• Documents accurately and timely; maintains compliance with regulations.
• Participates in quality improvement projects and delivers excellent customer service.
• All other duties as assigned.

The Nurse Practitioner is responsible for the provision of preventive medical, palliative, and educational services to promote patient health and wellness and uses standard software and the comprehensive clinical-financial computer system ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations.

Essential Activities and Tasks

Care Management - 100%

• Conducts comprehensive medical and social histories and performs physical examination of healthy, acutely ill, chronically ill patients or terminal patients.
• Treats common acute illnesses, chronic and stable health conditions through prescribing medications and other treatments.
• Orders and interprets lab tests and x-rays.
• Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, palliative care and medication use.
• Refers patients to other health professionals as needed.
• Communicates with staff members, patients and families to promote optimal patient outcomes.
• Completes documentation accurately and timely while maintaining state, federal and agency regulation.
• Participates in quality improvement projects.
• Commitment to excellent customer service for internal and external customers.

All other duties as assigned.

Qualifications

Education

• Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner (CNP) required.
• Current Certificate of Prescriptive Authority or Externship Certificate to Prescribe.

Experience

• Two years experience in the home health and/or hospice field required.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
• Proficiency with electronic medical records (EMR) preferred.

Other Requirements

• Must be able to read, write, speak, and understand the English language.
• Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
