Job summary

AccentCare is seeking a Nurse RN IPC Shift Lead, Hospice to oversee daily clinical operations in Wilmington IPC. You will lead RNs, coordinate patient care, and ensure timely documentation while maintaining compliance with hospice standards.

The role demands an active RN license, CPR certification, and at least one year of supervisory experience in healthcare, with hospice/home health exposure preferred. On-site schedule with 3/12-hour shifts and weekend rotation.

Qualifications

• Must hold a current Registered Nurse license in the state of practice; associate degree preferred.
• Current CPR certification required in Georgia and California, preferred in other locations.
• Requires a minimum of one year of supervisory experience in a healthcare setting.
• Prior nursing experience in hospice, home health, med-surg, or specific patient populations; one year required in Connecticut.
• Must have a valid driver's license, reliable vehicle, and state-required liability auto insurance.
• Must demonstrate a strong commitment to our mission, values, and philosophy of care.

Responsibilities

• Coordinate daily clinical operations by assigning patient care responsibilities to RNs, managing admissions and transfers, and ensuring appropriate patient rounding and documentation.
• Support staff performance through coaching, Back 2 Bedside visits, chart audits, medication management oversight, and compliance checks.
• Maintain ongoing communication with the Team Director, Director of Clinical Operations, physicians, and interdisciplinary team to align care plans, staffing, and patient needs.
• Ensure patient and caregiver education is provided, covering medication use, personal care, mobility, and end-of-life preparation.
• Oversee accurate, timely patient record-keeping and care planning, ensuring documentation is completed before staff leave the inpatient center each shift.
• Participate in meetings, performance reviews, staff training, orientation, and special projects, while staying current with hospice regulations and agency policies.
