Job summary

Hartford Healthcare is seeking a compassionate Hospice Nurse to join our Hartford-based hospice team. In this role you will provide high-quality, patient-centered end-of-life care in homes and affiliated facilities, completing thorough assessments, managing pain and symptoms, and developing individualized care plans.

You will coordinate closely with physicians, social workers, chaplains, and caregivers to support patients and families through physical, emotional, and spiritual needs.

Qualifications

• Valid RN license in New York; hospice experience preferred.
• Ability to perform thorough patient assessments and manage end-of-life symptoms.
• Experience coordinating care with physicians, social workers, and chaplains.
• Comfort with educating families and documenting in the EHR.

Responsibilities

• Provide patient-centered end-of-life care in homes and affiliated facilities.
• Assess pain and symptoms, administer medications, and adjust care plans.
