Job summary

Agape Care Group is seeking a Hospice Nurse Practitioner in South Carolina to provide diagnosis, treatment, and comprehensive palliative care under physician supervision. You will support families through health care journeys with compassion while joining a leadership team committed to professional development and advancement.

The role emphasizes a patient- and family-centered approach in hospice and palliative care, with strong emphasis on integrity, collaboration, and ongoing growth

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
• Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred.
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.

• Provide patients with diagnosis, treatment, and care under the supervision of a physician.
• Focus on providing palliative care and assist families along their health care journeys with love and compassion.
