Job summary

Shalom Hospice is seeking a Board Certified Music Therapist (or eligible to take the certification exam) to provide music therapy services as part of a patient-centered care plan. This role supports physical, emotional, cognitive and social needs within a hospice setting and reports to the Clinical Manager.

You will assess patients, direct therapy, educate family members and participate in interdisciplinary teams while maintaining CPR certification and state licensure requirements.

Qualifications

• Requires a degree in music therapy from an accredited program.
• Certified to practice as a music therapist within the state.

Responsibilities

• Provide music therapy services per a written plan of care.
• Assess and evaluate therapeutic status and adjust care plan.
• Direct music therapy treatment and determine priority needs.
• Prepare and submit clinical progress summaries.
• Provide music therapy consultation to families/caregivers.
• In-service education for nursing staff.
• Participate in interdisciplinary team and quality improvement activities.
• Complete orientation for hospice team members.
