Job summary

AdvisaCare Home Health and Hospice is seeking a full-time MSW to join our compassionate hospice team in Lakeview, OR. You will perform psychosocial assessments, support goal-directed care planning, and collaborate with physicians and the interdisciplinary team.

You will document visits and referrals, provide crisis and bereavement counseling, and advocate for patient and family needs while adhering to hospice policies.

Qualifications

• MSW degree required.

Responsibilities

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in developing goal-directed care plans.
• Documents patient visits, telephone contacts and referrals in the patient record.
• Conducts reassessments of patient and family needs and counseling as required.
• Provides crisis intervention and individual or family counseling when indicated.
• Participates as a member of the Bereavement Team as assigned.
• Adheres to Hospice policies and advocates for patient/family needs.
• Develops and maintains contacts with community services to facilitate referrals.
