Job summary

Residential Home Health and Hospice is hiring a Medical Social Worker (MSW) for a hospice role delivering in-home care to patients in the Naperville, IL area and surrounding communities. You will conduct psychosocial assessments, support patients and families, and coordinate end-of-life planning with an interdisciplinary team.

The position requires an MSW from an accredited program, valid state MSW licensure, and the ability to travel ~20% of work time to patient locations.

Qualifications

• MSW from graduate program accredited by the Council on Social Work Education.
• Must hold a state-licensed Medical Social Worker credential in good standing.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Conduct psychosocial and bereavement assessments; develop care plans with emotional and social goals.
• Provide counseling and guidance to patients and families; connect to resources for end-of-life planning.
• Apply critical thinking to address complex cases; document accurately in EMR systems.
• Work independently while collaborating with an interdisciplinary team.
• Travel to patient homes within the assigned territory.

MSW Qualifications

• Master’s in Social Work from graduate school accredited by the Council of Social Work
• One (1) to two (2) years post graduate social work experience in a health care setting
• Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
