Job summary

At Home Hospice, part of LHC Group within the Optum family, is seeking a Social Worker to support medical social services under physician direction and an interdisciplinary group. This role focuses on addressing social and emotional factors affecting patient health and coping strategies in a hospice context.

The position emphasizes collaboration to create a comprehensive Plan of Care, navigation of community resources, and ongoing interaction with patients, families, and health care teams.

Qualifications

• Master's Degree from a CSWE-accredited program.
• Current Driver's License and access to a dependable vehicle or public transportation.
• CO licensure as a Social Worker.

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis

As the Social Worker, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms.

Primary Responsibilities:

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instructs health care team members on community resources available to assist patients on a as needed basis

Required Qualifications:

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Additional State Specific Requirement:

• CO: Licensed as a Social Worker from the State Board of Social Work.

Preferred Qualifications:

• Current CPR certification or ability to complete within 90 days of hire
• Licensed Social Worker in the state of residence
• 1+ years of social work experience in a health care setting

The salary for this role will range from $73,040 to $109,560 annually based on full-time employment.
