Job summary

Hackensack Meridian Health in Wall, New Jersey, is seeking an Associate Medical Director of Hospice to lead clinical care and shape hospice standards. The role partners with the Medical Director, attending physicians, and the IDG to ensure seamless continuity of care and expert symptom management for patients and families facing end-of-life decisions.

Qualifications

• Doctorate Degree in Medicine (MD) or Osteopathy (DO).
• Minimum of two (2) years of experience beyond board certification.
• At least three (3) years of general healthcare experience.
• A current state medical license in good standing with the medical board.

Responsibilities

• Direct Patient Care: Step in as the primary or designee physician when an attending physician is unavailable, ensuring 24/7 availability.
• Clinical Leadership: Consult on pain and symptom management, establish care plans, and provide certifications and narratives.
• Program Strategy: Drive clinical outcomes including reducing hospital readmissions and optimizing hospice utilization.
• Relationship Building: Partner with community physicians, health plans, and donors to coordinate care and referrals.
• Education & Academics: Train hospice staff and teach at the School of Medicine; lead team meetings and case reviews.
• On-Call: Participate in a shared, rotational after-hours schedule to ensure continuous care.
