Job summary

Northgate Health Care Facility, part of The McGuire Group, is seeking an LPN Unit Coordinator of Hospice to lead a nursing unit and ensure high-quality resident care. You will oversee daily clinical operations, supervise staff, and coordinate with physicians, families, and interdisciplinary teams.

Qualified candidates will hold an NYS LPN license with at least 1 year in long-term care and have supervisory experience. Sign-on bonus and comprehensive benefits are provided.

Qualifications

• High School Diploma or equivalent required.
• NYS Licensed Practical Nurse (LPN) license required.
• Minimum 1 year LPN experience in long-term care.
• 3–5 years LTC experience with supervisory responsibility preferred.
• Strong knowledge of resident care planning, clinical documentation, and LTC regulations.

Responsibilities

• Oversee the clinical care and daily operations of the Hospice unit.
• Supervise nursing staff, nursing assistants, and unit support staff with leadership.
• Monitor resident conditions, review assessments, and ensure updated care plans.
• Coordinate communication with physicians, nurse practitioners, PAs, residents, and families.
• Review labs, orders, incidents, and changes in residents’ condition.
• Participate in care plan meetings, QAPI activities, and interdisciplinary discussions.
• Assist with admissions, discharges, transfers, and outside appointments coordination.
• Ensure unit supplies and resident records are accurate and up-to-date.
• Support staff education, infection prevention efforts, and quality improvement.
• Address resident, family, and staff concerns to promote a positive care environment.
