Responsibilities

• Schedule and coordinate patient visits with the hospice team.
• Develop individualized care plans with the team.
• Provide physical, emotional, and spiritual support to patients and families.

• Maintain accurate records of patient care and interactions.
• Collaborate to identify patient needs and provide education as needed.
