Job summary

Qualifications

• Must be licensed as LPN/LVN in Utah.
• Three years nursing experience, hospice or medical/surgical preferred.
• Flexible in work hours and willing to maintain knowledge of information systems.

Responsibilities

• Provide direct patient care per State Nurse Practice Act.
• Implement current nursing practice with the plan of care.
• Document patient services accurately and timely.
• Educate patient and family about diagnosis and self-care plan.
• Coordinate hospice services with interdisciplinary team.
• Report identified needs to RN/Clinical Supervisor and participate in QA activities.
• Perform other duties as assigned by the registered nurse.
