Qualifications

• Minimum of one year experience as an LPN in Med/surg nursing and one year in Hospice or Oncology Nursing.
• Driver's license and car required for all visits except in Manhattan.

Responsibilities

• LPN visits hospice patients in their home or skilled nursing facility.
• Provides direct care and documents each visit using the Netsmart EMR system.
