Job summary

NEXT CARE HOSPICE is seeking a Per Diem / Part-time LVN in the Orange County region to deliver high-quality hospice care to assigned patients.

The role emphasizes compassionate patient care, building rapport with patients and families, and ensuring proper medication management, with travel required to patient sites. California LVN license and hospice/home health experience are preferred.

Qualifications

• Valid LVN license in California.
• One year or more of experience in either Home Health or Hospice is preferred.
• Previous hospice experience is a plus.

Responsibilities

• Provide Skilled Nursing Care in a home or facility setting.
• Ensure the appropriate use of meds, DME, and supplies.
• Educate and support patients, families, and caregivers.
• Provide our patients and families the best care possible.
• Travel is required when working.
