Job summary

AdvisaCare Hospice is seeking a compassionate LPN to join our Troy-based team serving Grand Rapids and surrounding areas. You will provide 1-on-1 patient care, emotional support, and work within an interdisciplinary team to honor patients’ final days with dignity.

You must hold a Michigan LPN license, have hospice experience, and be flexible for on-call shifts. This role emphasizes real conversations, comfort, and meaningful connections with patients and families.

Qualifications

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily, weekly office meetings as indicated.
• Ability to adapt to patient’s status and needs on a full time on call basis.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance.
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern.
• Supervision of paraprofessionals providing services to patient.
• Preparing clinical and progress notes, participating in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with our clients, families and caregivers.
• Communication of information using current process and technology within organization.

We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Community with our Troy office location being your Parent Agency servicing our clients for the Grand Rapids and surrounding areas!

For this role, you need to be a Licensed Practical Nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.
