Providence at Home with Compassus seeks an Intake Assessment Nurse LVN for an in-office role in Santa Rosa. You will accept referrals by phone and fax, coordinate with clinicians, and ensure compliant intake processes.
The position requires California LVN licensure and 1 year each of acute care and home health experience, with strong communication and organizational skills. Working hours are 8am–4:30pm PST, in-office.

An Intake assessment Nurse LVN is a LVN with home health experience whose primary responsibility is to accept referrals from a variety of resources via phone and fax.
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
Language Skills: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
Computer Skills: Knowledge of computers.
Other Skills and Abilities: Excellent communications skills.
Other Skills and Abilities: Mandatory interpersonal and communication skills.
Other Skills and Abilities: Excellent customer service skills.

Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $34.90 - $53.39 The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
