McLeod Hospice Homecare is seeking a dedicated Registered Nurse (RN) to provide compassionate end-of-life care in patients' homes in the Florence and Cheraw areas. The role includes assessment, care planning, and acting as primary case manager within an interdisciplinary hospice team.

You will participate in on-call rotations to ensure 24/7 nursing coverage and deliver family education, while upholding McLeod Health's Service Excellence Standards and Core Values.

Formación

• Active patient care experience within the past year.
• Current South Carolina RN license.
• Minimum of one year of nursing experience.
• Refresher course within the past year if applicable.
• SC Driver's License.
• Automobile insurance as required by law.
• BLS certification through the American Heart Association.

Responsabilidades

• Assess patients through referrals, nursing assessments, and diagnostic review.
• Develop and implement individualized care plans per physician orders.
• Provide skilled nursing care per care plans.
• Serve as primary case manager coordinating the hospice team.
• Communicate patient status in team meetings and with leadership.
• Coordinate inpatient, respite, continuous, and routine home care services when needed.
• Participate in 24/7 on-call nursing coverage.
• Maintain accurate, timely documentation per policies.
• Complete required education, training, and competency development.
• Deliver age-appropriate, culturally sensitive care and family education.
• Demonstrate McLeod Health's Service Excellence Standards and Core Values.
