Qualifications

• Experience in home health or skilled nursing settings.
• Ability to perform OASIS documentation and state/federal regulatory requirements.
• Strong clinical assessment and care planning capabilities.

Responsibilities

• Provide hospice coverage when on-call and manage caseloads.
• Complete and maintain accurate OASIS documentation.
• Develop and communicate plans of care with patient and family.
• Act as Case Manager for assigned patients and coordinate visits.
• Implement plan of care and nursing interventions.
• Maintain up-to-date documentation of care provided.
