Job summary

TRI-COUNTY HEALTHCARE in McAllen, TX is seeking an LVN to provide in-home hospice and home health care under RN supervision. You will deliver nursing care, assess patients, and document care per physician orders.

The role offers part-time or PRN shifts with travel to patient homes, competitive pay, and benefits. Must hold LVN licensure, BLS, a valid driver’s license, and have 2+ years of LVN experience.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current state LVN License.
• BLS Certification required.
• Valid Driver’s License.
• Minimum 2 years LVN experience in clinical, hospice, or home health settings (preferred).

Responsibilities

• Provide direct nursing care to patients in their homes and follow physician orders.
• Regularly monitor vital signs, document changes, and maintain health records.
• Monitor patients’ medications and ensure proper administration.
• Collaborate with the interdisciplinary team, patients and families on care plans.
• Assist with emotional, personal and medical support to meet patient needs.
• Evaluate patient behavior and consult with providers to improve outcomes.
