Job summary

Mercy is seeking a dedicated Manager to lead Hospice and Home Health Intake operations across Mercy Ministries, ensuring a consistent, efficient, and high-quality experience for patients, families, referral partners, and caregivers.

This role will guide East region Hospice Intake initially, drive standardization, caregiver engagement, and resource optimization, with plans to expand to Home Health Intake, while partnering with leaders, physicians, and the health system to sustain growth in

Qualifications

• Graduate from a school of nursing is required.
• BSN preferred.
• 3 years of nursing experience.
• Previous leadership/management experience preferred.
• Active RN license in state of practice is required.
• BLS Certification required within 90 days of hire.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• Demonstrates skill in human relations and communications.

Responsibilities

• Lead and coordinate Hospice and Home Health Intake operations across Mercy Ministries, ensuring a consistent, efficient, and high-quality experience for patients, families, referral partners, and caregivers.
• Initially oversee Hospice Intake for the East region and align Intake services across additional Hospice communities, with future expansion to Home Health Intake.
• Drive process standardization, optimize resource utilization, and monitor operational, quality, and service metrics to meet productivity/financial goals.
• Collaborate with leaders, physicians, referral sources, and caregivers across Hospice, Home Health, and the broader health system to improve communication and streamline workflows.
