Job summary

Choice Health At Home is seeking a Hospice Field LVN/LPN PRN in Overland Park, KS. The role supports end-of-life care in patients'' homes or care facilities, focusing on symptom management, comfort, and quality of life under RN supervision.

Qualifications

Current LVN/LPN license is required.
Valid driver''s license is required.
Valid auto insurance is required.
CPR certification is required.
HomeCare HomeBase experience is preferred.

Responsibilities

Under the direction of the RN, identify the patient''s physical, psycho-social, and environmental needs and document findings.
Plan and implement care in conjunction with the RN per the plan of care.
Apply infection control concepts and standard precautions to care activities.
Ensure continuity of care through delivery of quality patient care.
Provide effective communication to patient/family, team members, and other health care professionals.
Complete clinical and progress notes documenting intervention outcomes.
Assess patient/family response to care and communicate findings to the team.
