Job summary

Devotion Hospice in Kingfisher, OK seeks a Hospice Field LPN PRN to provide compassionate, patient-centered care in home or facility settings, focusing on symptom management and comfort during end‑of‑life care.

Under the supervision of the RN, you will help identify patient needs, plan and implement care per the POC, and apply infection control to ensure safety. You will communicate with families and the care team and document outcomes for ongoing care decisions.

Responsibilities

• Under the direction of the RN, identify the patient's physical, psychosocial, and environmental needs as evidenced by documentation, clinical records, team reports, and evaluations.
• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Assure the continuity of care through delivery of quality patient care.
• Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Complete appropriate clinical and progress notes, documenting outcomes of interventions.
• Assess patient/family response to care, ensuring communication of information to appropriate team members.

The Hospice Field LVN/LPN plays a critical role in providing compassionate, patient-centered care to individuals facing life-limiting illnesses in their homes or care facilities. This position focuses on managing and alleviating symptoms, supporting patients' comfort, and enhancing their quality of life during end-of-life care. The LVN/LPN collaborates closely with interdisciplinary hospice teams, including physicians, social workers, and chaplains, to develop and implement individualized care plans. Additionally, the role involves educating and supporting patients' families and caregivers, ensuring they are equipped to provide appropriate care and emotional support. Ultimately, the Hospice Field LVN/LPN serves as a vital advocate for patients' dignity and comfort throughout their hospice journey.

As a Hospice LPN , you will:
