Job summary

Magnolia Hospice seeks a Hospice Education Specialist to expand reach by building relationships with referral sources, educating the community, and representing Magnolia Hospice with professionalism and compassion.

You will collaborate with clinical teams, help ensure smooth transitions from referral to admission, and strive to meet monthly referral and admission goals while maintaining HIPAA compliance.

Qualifications

• Education/Experience in sales, marketing, or business development in home care or hospice is preferred.
• Ability to read, write, and carry out directions withAccuracy.
• Knowledge of home health regulation and referral processes.

Responsibilities

• Referral Source Development: Build and maintain relationships with physicians, hospitals, SNFs, assisted living facilities, and other referral partners; conduct visits and calls to promote awareness.
• Community Engagement: Represent Magnolia Hospice at health fairs and events; deliver presentations to healthcare professionals and community leaders.
• Education & Advocacy: Communicate eligibility criteria and Magnolia Hospice’s approach; advocate for patient access to services.
• Collaboration with Clinical Teams: Work with intake and clinical teams to ensure smooth referrals and admissions; provide market feedback to leadership.
• Additional Responsibilities: Perform other duties; maintain confidentiality; participate in ongoing education.

Job Summary:

As a Hospice Education Specialist, you will help expand our reach and connect more patients and families with the care they deserve by building relationships with healthcare referral sources, educating the community, and representing Magnolia Hospice with professionalism and compassion.

Essential Functions:

Referral Source Development

• Build and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners.
• Conduct regular visits and calls to referral sources to promote awareness of home health services.

Community Engagement

• Represent Magnolia Hospice at health fairs, community events, and networking opportunities.
• Deliver presentations to healthcare professionals, community leaders, and organizations to build awareness and understanding of the benefits of timely hospice care.

Education & Advocacy

• Communicate eligibility criteria, home health philosophy, and Magnolia Hospice’s personalized approach to care.
• Advocate for patient and family needs by ensuring access to services when appropriate.

Collaboration with Clinical Teams

• Work alongside intake and clinical teams to ensure smooth transitions from referral to admission.
• Provide market feedback to leadership to support service quality and growth strategies.
• Performance Accountability
• Meet or exceed monthly referral and admissions goals.
• Document all contacts, meetings, and activities in the company CRM in compliance with HIPAA and policy guidelines.

Additional Responsibilities:

• Performs other related duties as assigned or requested
• Conforms to all applicable Agency policies and procedures.
• Participates actively in continuing education and in-services.
• Maintains confidentiality of patient information and business trade practices.
• Assumes accountability for reporting incidents and complaints according to Agency policy.

Minimum Position Qualifications:

Education: High school diploma or equivalent; Bachelor’s degree preferred

Experience: One (1) to three (3) years’ experience in sales, marketing, or business development experience, home care or hospice, preferred

License / Certification: Driver’s license and proof of current auto liability insurance; No Listing in the OIG Excluded Provider Listing; listed in good standing on state nurse aide registry or requirements of a state licensure program meeting federal regulations.

Environmental Conditions:

Works in patient's home in various conditions, possible exposure to blood and body fluids and infectious diseases. Ability to work flexible schedule, ability to travel locally, some exposure to unpleasant weather. Moderate noise level; Category A BBP risk; moderate stress and emotional demands.

Physical Requirements:

Prolonged standing and walking required. Ability to move up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions and notify a supervisor. Requires hand-eye coordination and manual dexterity. Ability to utilize durable medical equipment in the home.
