Job summary

Elite Hospice, part of the Optum family, is seeking a Hospice Home and Community Based Services Aide to provide personal care and support to clients at home, enabling them to stay in their own residence. You will assist with daily living activities, monitor health conditions, and document care using EVV.

The role requires a Texas Home Health Aide background with 1+ year of relevant experience, CPR certification, and the ability to work flexible hours independently.

Qualifications

• TX Home Health Aide: minimum 1 year full-time in hospital or nursing facility, or 1 year direct client care in agency setting, or approved training/competency program within last 24 months, or nursing student with appropriate documentation.
• Must have current CPR certification or complete within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal prep, and accompany clients to appointments or errands.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Work with clients of diverse backgrounds and treat them with dignity and respect.

As the Hospice Home and Community Bases Services Aide, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home.

Primary Responsibilities:

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualification:

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation

• TX: Home Health Aide-Must have a minimum of one-(1) year full-time experience in hospital or nursing facility, OR one (1) year full- time experience within last five (5) years in direct client care in agency setting, OR documented completion of training and competency evaluation program with home health experience within the last twenty-four (24) months, OR documented completion of competency evaluation program with home health experience within the last twenty-four (24) months, OR nursing student with documentation from director of programs or the dean of a school of nursing that states that the individual is a nursing student who has demonstrated competency in basic nursing skills aligned with the school's curriculum, OR be on Texas Department of Human Services (DHS's) Nurse Aide Registry with no findings of client abuse or neglect or misappropriation of client property. Must NOT be listed on the Employee Misconduct Registry.
