CorsoCare Hospice is seeking a Hospice Aide (PRN) to provide in-home personal care and support to patients in Hamilton County, IN. The role reports to the Clinical Manager and works with the health services team under supervision of a Staff RN/Case Manager.
Requirements include Indiana CNA certification and at least 1 year hospice experience; CPR is preferred. A valid driver’s license with reliable transportation is necessary. Strong communication and team collaboration are essential.

The Hospice Aide reports to the Clinical Manager and is expected to function as a member of the health services team The Home Health Aide is trained to provide personal care and related services in the home The Hospice Aide is assigned to specific patients and functions under the direction instruction and supervision of the Staff Registered NurseCase Manager the Supervising Therapist and Clinical Manager
Currently Certified as a CNA in the State of Indiana Minimum of 1 year of Hospice Aide experience in a health care or home care settingCPR certification preferred but not requiredCurrent drivers license with current automobile insurance and must have reliable transportationExcellent communications skillsCompliant with accepted standards and practicesMust be able to meet the physical demands of providing patient care ambulating patients transferring patients assisting with exercises etcSelf directed with the ability to work with little supervisionAvid team playerConsistently maintains a positive attitude which promotes team and optimal performanceMust be empathetic nonjudgmental tactful responsible adaptable organized and committed to the purpose and objectives of the Agency
Assist in maintaining a clean healthful and safe environmentAssist the patient with ambulation Uses safe transfer and ambulation techniquesAssist with nourishment and fluid needs including prepared meals feeding patient and offering fluidsReading and recording temperature pulse and respirationMaintains basic infection control proceduresAdherence to the Hospice Aide care planGives simple emotional and psychological support to the patient and other members of the householdEstablishes a relationship with patient and family which transmits trust and confidentialityResponsibility for Hospice Aide Contacts the Clinical Manager to inform himher of alternate schedule for patient visits during vacation sick days or personal days off Reports any schedule change or inability to complete assignment as scheduledAccepts changesadditions to schedule during staffing difficulties when necessary Accepts assignment in time of emergency regardless of any location andor any day of the week be it weekendholidaysCompliance with all applicable laws regulatory requirements standards of practice accreditation standards and policies and proceduresOrganizes daily visit schedule to minimize travel time and mileage Maintains productivity according to work statusMaintains OSHA and Infection control per policyPerforms other duties as assigned
This position entails standing for long periods of time While performing the duties of this job the employee is required to communicate effectively with others sit stand walk and use hands to handle keyboard telephone paper files and other equipment and objects The employee is occasionally required to reach with hands and arms This position requires the ability to review detailed documents and read computer screens The employee will occasionally lift andor move up to 25 pounds The work environment requires appropriate interaction with others The noise level in the work environment is moderate Occasional travel to different locations may be required
