JourneyCare Hospice is seeking a Certified Nurse Aide (CNA) to join our compassionate team in Chicago, IL. This role focuses on delivering high-quality personal care and comfort to hospice patients in their homes, with a Monday–Friday day schedule and no weekend shifts.

Qualified CNAs must hold active certification, CPR, and a clear registry record. You will work under the Case Manager, providing empathetic, patient-centered support, and collaborating with licensed staff to ensure safety and

Qualifications

• Current CNA certification per state requirements.
• CPR certification maintained.
• Listed on state aide registry with a clear record.
• 18 years of age or older (as required by state).
• Ability to read and follow written instructions and document care.
• Comfort with hospice philosophy and terminally ill patients.
• Self-directed with minimal supervision.
• Comfortable with death/dying.
• Calm, empathetic demeanor in patient homes.
• Strong oral and written communication; good organizational skills.
• Valid driver’s license with insured vehicle or reliable transportation.

Responsabilités

• Perform personal care tasks after patient assignment and respond to requests.
• Ensure safety and proper use of equipment and supplies.
• Provide bathing, grooming, oral care, shampoos and bed linens changes as assigned.
• Assist patients with dressing and undressing.
• Plan and prepare nutritious meals and assist with feeding as directed.
• Take and record vital signs when ordered and demonstrate competency.
• Monitor skin integrity and report changes to the Case Manager.
• Assist with ambulation and prescribed exercises.
• Perform range-of-motion or other simple procedures as ordered.
• Provide respite to patient, family, or caregiver when on-site.
• Respect patient and family environment and privacy.
• Maintain confidentiality at all times.
• Perform other duties as assigned.
