Job summary

Hospice of the Midwest in Woodbury, MN is seeking a dedicated Hospice CNA to join our care team. You will provide 1:1 patient care, support families, and help maintain a dignified home setting for our patients.

Qualifications

• Six months of experience including providing personal care.
• Must be 18+ years old.
• CPR certification current where required.

Responsibilities

• Provide 1:1 care to patients and families.
• Monitor and report changes in patient conditions.
• Assist with mobility, bathing, dressing, and daily activities.
• Support with medication management as allowed by state regulations.
• Maintain accurate documentation and collaborate with hospice team.
