Job summary

Adoration Health in Murfreesboro, TN is seeking a dedicated Hospice CNA to join our team. You will provide dignity, comfort, and supportive care at a critical time in patients' lives within a home-based hospice setting.

Responsibilities include personal care, mobility assistance, medication help per state rules, and strong collaboration with the hospice interdisciplinary team. Training requirements include CPR certification and state-specific CNA qualifications.

Qualifications

• Minimum six months experience providing personal care
• State-required training and CPR certification
• Ability to read and follow written instructions and document care

Responsibilities

• Provide 1:1 patient care including bathing, dressing, toileting, mobility and repositioning
• Monitor patient condition and report changes to the team
• Assist with ambulation, transfers, ROM exercises and safe equipment use
• Help patients with self-administered medications per state regulations
• Offer companionship and emotional support to patients and families
• Light housekeeping and meal assistance; maintain a clean space
• Maintain accurate and timely documentation of care
• Collaborate with hospice interdisciplinary team
