Job summary

Hospice of the Midwest is seeking a dedicated Hospice CNA to join our Forest Lake team, providing 1:1 care to patients in a compassionate, dignified setting. The role emphasizes independence with strong team support and a schedule Monday–Friday, 8:00 AM–5:00 PM in Forest Lake, MN.

Responsibilities include personal care, monitoring patient changes, mobility assistance, and medication help within state regulations. We offer comprehensive benefits and a stable, growing healthcare provider.

Qualifications

• Six months CNA experience including personal care.
• CPR certification and ability to read/write care documentation.
• Comfortable working with terminally ill patients within hospice guidelines.
• 18+ years old and able to work independently with team support.
• Demonstrates strong communication and organizational skills.

Responsibilities

• Personal Care: bathing, dressing, toileting, mobility, repositioning.
• Monitor & Report: observe changes and communicate with team.
• Mobility: assist with ambulation, transfers, ROM, equipment safety.
• Medication Assistance: help with self-administered meds per state rules.
• Emotional Support: provide companionship to patients and families.
• Light housekeeping: tidy space, assist with meals.
• Documentation: maintain accurate patient records.
• Team Participation: collaborate with interdisciplinary team.
