Job summary

Geisinger Hospice, part of the Optum family, is seeking a Home and Community Based Services Aide to assist individuals at home with personal hygiene and daily living activities. You will help with bathing, grooming, meal prep, and accompany clients to appointments while monitoring conditions and documenting vital signs.

PA-specific training is required or provided, with CPR preferred and at least 6+ months in home care.

Qualifications

• Must possess a valid PA driver’s license and reliable transportation.
• Current CPR certification preferred or ability to obtain within 90 days.
• Experience providing personal care and support in home settings is advantageous.
• Ability to work independently and adapt to flexible scheduling.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing and ambulation.
• Support household tasks, meal preparation, and accompany clients to appointments or errands.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and EVV usage.
• Ensure a safe home environment and properly operate medical equipment; respond promptly to client needs.
• Demonstrate respect for all clients regardless of age, race, creed, color, sex, disability, or financial condition.

As the Home and Community Bases Services Aide, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home.

Primary Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualification

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation

State-Specific Requirement

• PA: Successfully complete one of the following: a competency OR training program developed by the agency that meets the requirements identified in 28 Pa. Code § 611.55 OR A home health aide training program meeting the requirements of 42 CFR 484.36 OR A training program meeting the training standards imposed on the agency as a provider in a Medicaid Waiver or other publicly funded program providing PCS services.

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire
• 6+ months of home care experience
• Ability to work flexible hours and independently

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $14.00 to $23.85 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
