Job summary

Bloom Healthcare in Fort Collins, CO seeks a Hospice Clinical Supervisor to lead clinical teams delivering home-based hospice care. You will assess cases, guide clinicians, and develop care plans to improve quality of life.

Qualifications

• Holds a valid Colorado State Nursing License as a Registered Nurse.
• Has a minimum of three years of clinical nursing experience, specializing in end-of-life care.
• Prior experience in clinical leadership or supervisory roles REQUIRED.
• Prior experience with CHAP, ACHC or JCAHO REQUIRED.
• Capable of self-direction and autonomous work.
• Skilled in clinical coordination and communication.

Responsibilities

• Conducts rigorous clinical assessments of cases, evaluating diagnosis, prognosis, pharmacotherapy, and overall trajectory.
• Offers structured guidance to clinicians to uphold quality standards.
• Partners with clinical staff for care goal setting, prioritization, and comprehensive planning.
• Oversees or assists in clinical staff performance evaluations.
• Chairs or partakes in clinical case conferences, promoting interdisciplinary collaboration.
• Spearheads the development and refinement of clinical protocols, guidelines, and organizational policies.
• Strictly adheres to professional clinical standards and ethical guidelines.
• Advocates for a customer-centric approach within the clinical team.
• Engages in clinical quality assurance and performance improvement, providing leadership and expertise.
• Commits to ongoing professional development through clinical workshops and participation in clinical networks or societies.
• Undertakes additional tasks as assigned by the Director of Clinical Services.

The pay range for this role in the state of Colorado typically falls between $110,000 - $125,000with the potential for performance-based bonuses and other benefits.
