Job summary

Ohio Living is seeking a Hospice Clinical Supervisor to lead the clinical team in delivering compassionate end-of-life care. You will coordinate care plans, supervise IDG activities, and ensure compliance with state and federal standards while optimizing resource use and data quality.

The role requires an Ohio RN license with at least three years in clinical operations supervision and one year in hospice or Medicare-certified home health settings.

Qualifications

  • Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
  • Three years experience in clinical operations supervision.
  • One year experience in a hospice setting.
  • One year experience in a Medicare-certified home health agency required.
  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
  • Proficiency with electronic medical records (EMR) preferred.
  • Must be able to read, write, speak, and understand English.
  • Must possess a valid driver's license and acceptable driving record.

Responsibilities

  • Coordinate and oversee direct and indirect patient/family care services with comprehensive care plans.
  • Oversee the interdisciplinary group (IDG) reviews and revise the plan of care.
  • Organize IDG meetings at least every 15 days.
  • Interpret operational indicators to detect census changes and staffing needs.
  • Review diagnosis coding and plan of care orders per Medicare regulations.
  • Ensure equipment and monitoring meet quality and compliance standards.
  • Collaborate with marketing to manage referrals and patient needs.
  • Manage local EMR component and train staff on proper use.
  • Evaluate the plan of care effectiveness and staff performance.
  • Evaluate workflow, administrative, and departmental processes.
  • Monitor hospice revenue and collections and ensure accurate documentation.
  • Conduct supervised patient visits and participate in on-call rotation.
  • May assume Alternate Administrator duties during absences.

The Hospice Clinical Supervisor leads the hospice clinical team to utilize a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet hospice patient/family physical, emotional, psychosocial, and spiritual needs while coordinating available resources to promote quality and cost-effective outcomes through ongoing audits of medical records and clinical documentation to continually improve data quality in accordance with company standards and federal, state, and local standards, guidelines, and regulations

Essential Activities and Tasks
Operations Management and Administration - 55%
  • Facilitates clinical team with the coordination and oversight of all direct and indirect patient/family care services ensuring accurate evaluation of patient/family physical, emotional, psychosocial and spiritual needs, and development and implementation of comprehensive care plans that address those needs by following current hospice nursing professional standards of practice and agency approved protocols. Supervises the implementation of the physician ordered hospice plan of care, by coordinating, auditing, overseeing, integrating, and modifying the IDG resources necessary to reach desired outcomes.

  • Oversees Hospice IDG review, revision, and documentation of the individualized plan of care as frequently as the patient’s condition requires, but no less frequently than every 15 days.

  • Organizes and oversees hospice IDG (interdisciplinary group) meetings which are held no less frequently than every15 days.

  • Interprets operational indicators to detect census changes and increases/decreases in volume that could drive staffing levels, revenues, and/or expenses.

  • Evaluates and approves the clinical documentation and approves electronic medical record workflow according to established standards.

  • Reviews diagnosis coding and plan of care orders for all hospice patients in accordance with Medicare regulations.

  • Assures all equipment, systems, and monitoring are in place to meet or exceed quality, customer service, utilization, and compliance standards in all company hospice programs.

  • Collaborates with marketing team to manage processes, protocols, and timeliness of all referrals to meet the comprehensive needs of the patients.

  • Manages the local component of the integrated electronic medical record and assures that the office and clinical staff are well trained and use the system productively and accurately according to company policies.

  • Evaluates the effectiveness of the plan of care for hospice care delivery and collaborates with hospice clinical supervisors and hospice staff on best practice and desired outcomes.

  • Evaluates workflow processes, administrative processes, and departmental processes.

  • Monitors and maintains hospice revenue cycles and collection cycles and ensures accuracy in documentation and coding.

  • Conducts supervised patient visits.

  • May be asked to case manage to meet business needs.

  • Participates in on-call rotation, as needed.

  • May assume the role of Alternate Administrator during the temporary absence and/or vacancy of the Administrator.

Quality and Compliance Management - 30%
  • Audits and maintains current and accurate medical records and clinical documentation in accordance with company standards and federal, state, and local standards, guidelines, and regulations.

  • Ensures action is taken to correct problems identified through clinical record review, clinical audits, and fiscal audits.

  • Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers.

  • Oversees the state survey processes, mock survey, and compliance audits. Maintains a state of readiness for survey by regulatory bodies.

  • Serves as a resource to clinical team for questions and/or concerns regarding federal and state regulations, approved accrediting bodies, and professional standard of practice.

People Management and Development - 15%
  • Supervises the hospice clinical team to operate within the allocated resources and budgeted goals, efficiencies and productivity standards.

  • Fosters a positive work environment that attracts, retains, and motivates employees.

  • Interviews, selects, and orients new direct reports.

  • Defines, establishes, and promotes performance expectations so direct reports understand and demonstrate excellence through high quality and consistent performance.

  • Evaluates job performance of direct reports in accordance with the company policy and procedure and provides developmental guidance to staff for improved performance and growth.

  • Monitors accountability with organization requirements prescribed by the Employee Handbook. Documents and engages employees who are routinely not observing these requirements in an effort to achieve compliance. Works with Human Resources when circumstances arise that necessitate further documentation and intervention.

  • Participates in the development, planning, scheduling, conducting, and monitoring of in-service training programs, on-the-job training, and orientation programs for departmental staff.

  • Ensures that there is a reasonable and sufficient plan for staffing should a direct report be absent or a direct reporting position be vacant.

  • Responds timely and thoughtfully to questions, requests, and work product from departmental staff that has been provided for review and approval.

Qualifications
Education
  • Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.

  • Bachelor of Science in Nursing (BSN) preferred.

Experience
  • Three years experience in clinical operations supervision required.

  • One year experience in a hospice setting required.

  • One year experience in a Medicare-certified home health agency required.

  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.

  • Proficiency with electronic medical records (EMR) preferred.

Other Requirements
  • Must be able to read, write, speak, and understand the English language.

  • Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
