Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Hospice Clinical Specialist in Chattanooga, TN, to oversee nursing management, documentation, and patient care planning. You will coordinate Start of Care workflows and participate in IDG meetings to ensure timely, compliant, and well-coordinated care for patients and families.

The ideal candidate is an experienced RN with strong documentation skills and knowledge of hospice philosophy, ready to work with

Qualifications

• Associate degree in Nursing required.
• Three years of practice as a Registered Nurse.
• Hospice experience preferred.
• Current Nursing License and knowledge of Hospice Philosophy of Care.

Responsibilities

• Provide clinical oversight for nursing management, documentation, and patient care planning.
• Process workflow after Start of Care visits to ensure continuity of care.
• Participate in IDG meetings to coordinate patient care plans.
• Review and adjust SOC and recertification visit schedules as needed.
• Maintain and manage clinical orders within the HCHB system.
