Health First is seeking a Hospice Clinical Social Worker (MSW) to coordinate psychosocial care for assigned patients in collaboration with the patient, family, and care team. You will assess needs, plan interventions, and support timely access to resources.

In this role you apply advanced psychosocial skills to improve patient outcomes, document in the EMR, and participate in end-of-life discussions. A detailed, compassionate approach and teamwork are essential.

Responsabilidades

• Conducts psychosocial assessment to screen for crisis, abuse, financial or environmental issues, collaborates with RN Case Manager and care team to facilitate continuity of care.
• Assesses and documents in EMR patient and family needs and works with the interdisciplinary team to identify psychosocial needs.
• Provides therapeutic or crisis intervention counseling with patients and families, supportive services for End of Life Issues, Terminal care.

POSITION SUMMARY

The Hospice Clinical Social Worker MSW utilizes advanced psychosocial skills to facilitate the coordination of care through assessment, planning, intervention and evaluation. The Hospice Clinical Social Worker MSW promotes and evaluates the effective utilization of resources using current knowledge, awareness of community services, and serving as a role model to achieve optimal clinical and resource outcomes

Primary Accountabilities

• Conducts psychosocial assessment to screen for patients/families at risk for crisis, abuse, financial or environmental issues, collaborates with Registered Nurse (RN) Case Manager, ancillary staff and licensed practitioners in facilitating continuity of care for the patient; ensuring to completes mandatory reporting processes as required by regulations and best practices.
• Assesses and documents in electronic medical record (EMR) patient and family needs and works with interdisciplinary team to identify possible psychosocial needs.
